Default Spelling Mistakes on DoD

I have come across a few spelling mistakes with DoD in the last few weeks:

1)Li To Me;
2)Boarwalk Empire

Why would these mistakes show up?

Basic quality control, someone names the file & does not bother to check it
The best I've seen was "SOC_021210_UEL_WIENVFCP_HL5"

We also have stuff like:
Soccer: FAPL Chelsea vs Bolton
Soccer: Liverpool v Bolton
No set format or consistency

Well these measures don't seem to be going very well, we now have "Soccer: BPL: "
Does each person randomly make up a new format each day?

You mos know on a bad fax an 'E' may look like a 'B'. I still believe SS is on faxes and 4:3.

Evane, please could you explain what systems have been put in place, and how simple inconsistencies like this occur everyday:
1st T20 Australia vs England
Australia vs England 2nd T20

Again, I just don't understand why this is so difficult to get right:

Cricket: ODI 1 South Africa vs India
Cricket; South Africa vs India 2nd ODI

This is obviously not being taken seriously at all

Merv, I seem to recall us going on for ages about consistent formatting, tags and the like. It fell on deaf ears then. There is no reason to suspect that these requests will be taken seriously now.
